Please enter the gross wages (amounts earned before taxes were withheld) paid to you in the quarters indicated below. If you were not paid wages during a given quarter, enter zero for that quarter. When entering your wages, do not enter dollar signs, decimal points, or cents.

You may use pay stubs and/or W2 forms to determine your wages. If you use the wage information on a W2 form, convert the yearly amount reported into quarterly amounts. Include wages paid for work performed in another state, for the federal government and for the military.
